“To Err is Human” – we are all human which means we all make mistakes. It’s time to accept this and help our teams grow by supporting them through errors as well as teaching them how to prevent adverse events. A culture of patient safety is one of shared values, beliefs, norms and procedure related to patient safety among members of an organization, unit or team. We must look at the environment where our teams work to prevent error from happening and withhold individual blame when an error does occur. We must look at the systems in place and improve or change them to avoid future adverse events.
